Our Process

Registration

When you determine you need Respite, you can fill out the Registration Form. This is not a guarantee of service, but rather a starting touch point to see if your child is eligible for services.

Phone Call/Zoom Call with Parents

Blue Sky will schedule a meeting to review your answers on your Registration Form and determine eligibility of services.

Intake Form

Once your child is eligible for services, you will receive our intake form to fill out along with Medical Forms, Policies & Procedures, and Service Contract Agreement, and payment agreement.

Respite

Respite Provided

Reporting & invoicing

Post Respite, each family will receive detailed notes. These notes are able to be shared with your child’s therapist and other care providers. A final invoice based off the level of care needed throughout the days of respite, will be provided along with the respite notes.
